Responsible for planning, coordinating, and directing all activities as they relate to the admissions of the hospice patients. Demonstrate good communication and public relations skills.
Essential Job Functions/Responsibilities:
Assists in all daily patient referral and intake operations including assisting in the establishment and implementation of intake policies.
Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
Assists in the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
Orders all DME for new admits and current patients.
Establishes and maintains positive working relationships with current and potential referral sources.
Ensures seamless transition of patients to home care by providing patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
Ensures maximum third-party reimbursement through coordination of insurance verification and authorization processes.
Builds and monitors community and customer perceptions of Hospice of Acadiana, Inc as a high-quality provider of services.
Maintains comprehensive working knowledge of Hospice of Acadiana, Inc contractual relationships and ensures that patients are admitted according to contract provisions.
Participates in quality assessment performance improvement teams and activities.
Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Hospice of Acadiana, Inc.
Sends out daily census reports to relevant stakeholders.
Orders comfort care kits (medications) for new admissions.
Assembles admission packets for Calcutta and routine home care hospice patients.
Compiles NH binders for new nursing home admissions.
Qualifications:
Position Qualifications:
Licensure/Certification/Registration: Currently licensed in Louisiana as an LPN.
Education: Graduate of an accredited nursing school.
Experience: Previous hospice experience preferred, but not required.
