Description:

• Manage the care, schedules, and staffing needs for 25-40 clients
• Conduct intakes and assessments of individual seeking services
• Ensure person-centered care plans are carried out by Direct SupportWorkers (DSWs) and that the clients' health and well-being aremaintained
• Supervise, train, evaluate, and provide direction to DSWs regardingall aspects of the specific client care
• Communicate frequently with clients, families, and other affiliates toanswer questions, resolve issues, and ensure satisfaction withservices.
• Travel to clients' homes for meetings and make visits quarterly,annually, and as needed.
• Document and maintain records of all client communications, careplans, medical changes, visit notes, or incidents.
• Comply with all program guidelines and meet requirementsestablished by LDH, Medicaid, and Caring Touch Home Care
• Participate in after-hours on-call rotation and serve as backup staffworking in-home with clients as needed
• Review weekly timesheets for ER visits, urgent care visits,hospitalizations, client health changes, and scheduled hours

Salary Range: $30k+ (Depends on Experience)

Qualifications:

• 1 year experience (Case Management) but willing to train the right candidate
• Satisfactorily pass all required and applicable State & Federal eligibility verifications including background and driving. etc.
• Strong computer skills including Microsoft Office, Word, and Excel
• Experience with AxisCare software (not required, but a plus)
• Able to operate office equipment
• Basic addition/subtraction and general analytical skills
• Valid driver's license, auto insurance, reliable transportation, and a reliable cell phone
