LEDA Virtual Job Fair

In addition to posting your vacancies on LEDA's website, positions are also distributed to a network of workforce centers and educational institutions as appropriate.





Position: Service Coordinator

The St. Joseph Shelter Service Coordinator is responsible for providing the residents of St. Joseph Shelter with service coordination. All employees are expected to learn and exhibit the core values of Catholic Charities of Acadiana as defined in the employee handbook and on the Catholic Charities of Acadiana as defined in the employee handbook and on the CCA website (catholiccharitiesacadiana.org/opportunities/).

Duties will include but are not limited to the following:

Provide Service Coordination
• Provide service coordination for residents living in St. Joseph Shelter
• Assist residents with the development and implementation of service plans which include the following components: education and employment assistance, counseling, budgeting, parenting classes, support groups, volunteer hours, housing placement and medical care along with any other services needed by the residents.
• Assist clients with obtaining, and maintaining mainstream benefits
• Maintain comprehensive and complete records for each client. Documentation will include intake evaluation forms, program contract, documentation of homelessness, picture identification, social security cards, Medicaid cards, proof of income, goals, individual service plans, client history, chronology of any criminal justice system information, progress notes, psychiatric, medical, vocational, educational assessments as appropriate
• Document all services provided in the homeless management information system within 24 hours of service delivery
• Provide client advocacy and referrals to outside agencies
• Ensure cleanliness of resident rooms, and provide life skills training
• Coordinate service plans with other social service providers
• Attend scheduled service coordination staffings
• Provide transportation
• Monitor medication of clients
• Maintain office area and facility in clean and orderly fashion
• Provide crisis prevention/intervention as needed
• Communicate regularly with Admissions Coordinator and Director of Client Services

Qualifications:
• Minimum of a high school diploma/GED, with two years of case management or related social service experience required. Bachelor’s degree preferred.
• Knowledge of homelessness and housing programs helpful
• Experience providing services individuals and families experiencing homelessness
• Exceptional problem resolution skills
• Exceptional oral and written communication skills
• Valid LA driver’s license and an insured motor vehicle in which to transport clients


Job Specifications
Medium: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. 100% of the time.
